With this plan, Guarantee Trust Life (aka GTL) will pay a daily benefit for each day you receive various home health care services such as:

• Skilled Nursing care, RN
•  General Nursing care, (LPN, LVN)
• Physical Therapy / Speech Therapy / Occupational Therapy
• Home Health Aid
• Chemotherapy Specialist
• Enterostomal Therapy
• Respirational Therapy
• Medical Social Services

To qualify for these benefits, a licensed Health Care Practitioner must certify the patient as having a Cognitive Impairment or the inability to perform at least two (2) of six (6) Activities of Daily Living without substantial assistance.
